Job Title: Care Manager Profession: Case Manager Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 3 years inpatient clinical experience License: Registered Nurse (RN) Certifications: Certified Care Manager Must-Have: - Bachelor of Science in Nursing (BSN) - At least 1 year of Care Management experience Description: The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs. The Care Manager will attend interdisciplinary rounds. The Care Manager will complete initial assessments. The Care Manager will facilitate the development of a plan of care in collaboration with the patient, family, caregiver, and healthcare team. The Care Manager will communicate consistently with medical teams and patients regarding the progress of care needs related to post-discharge care requirements, such as home health and inpatient rehabilitation. The Care Manager will interact with post-acute care referral sources to establish a plan.
