organized, empathetic, and motivated Care Coordinator to join our

supportive and mission-driven environment. About the Role As a Care
Coordinator, you will serve as a trusted advocate and central point
of connection for clients, families, providers, and community
resources. You will spend approximately half your week conducting
in-person client or care team meetings and the remainder
coordinating services, documentation, and follow-up. This position
supports individuals who are chronically ill or living with
neurological or mental health diagnoses. Many clients reside in
long-term care settings. A full-time caseload ranges from 25–50
clients Your work will help ensure each client’s safety, dignity,
and greatest degree of independence within the least restrictive
environment Key Responsibilities Promote timely access to
appropriate medical and supportive care Develop, implement, and
monitor individualized care plans Coordinate communication between
clients, families, hospitals, clinics, specialty providers, and
community partners Manage transitions of care and referrals Support
personal care needs (groceries, personal care items, durable
medical goods) Maintain complete and accurate electronic records
Identify at-risk clients and contribute to program improvement
efforts Reassess client status and adjust care plans as needs
evolve Qualifications 3–5 years of experience in clinical or
community-based care settings (reduction considered for licensed
professionals) Strong organizational skills and ability to maintain
accurate documentation Comfort using communication technologies and
electronic systems Knowledge of local healthcare and community
resources is a plus Valid driver’s license and reliable personal
vehicle required Schedule & Work Environment Full-time:
Monday–Friday, 8:00 a.m. – 5:00 p.m. (Part-time may be considered)
Travel throughout Vermont and New Hampshire required Remote work
