Job Description Physical Therapist – Adult Home Care | Per diem Location: Serving Chittenden & Grand Isle Counties, VT ? We are looking for therapists that are available weekdays. As a home health physical therapist, you’ll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff , providing comprehensive, patient-centered care. Case load territory is assigned after orientation.
